At Muscle IQ We Provide Premium Physical Therapy

We accept DMBA and EMI Insurance.  We accept TriCare and BlueCross/BlueShield.  We accept all Auto Insurances like Allstate, and Geico.  Call us Mon-Fri 8am to 6pm at 801-310-0851.  We will call your insurance and verify your physical therapy benefits for you.

We also accept most Medicare Insurance.​

We are not in Network with Select Health.  We are not in Network with PEHP.  We are not in Network with Medicaid.​
